Mary Medical Center, a member of Trinity Health Mid-Atlantic, is seeking a Manager to lead our Care Management team!
Job Description:
Assists in the implementation of the Utilization Management Plan and sits as an active member of the Committee.
Evaluates effectiveness of assigned process improvement activities resultant from the Committee.
Facilitates ongoing communication between Reimbursement Integrity RNs, Care Managers and Social Workers in support of efficient discharge planning and patient transition.
Coordinates and supervises complex discharge planning for St.
Mary Medical Center patients.
Recruits, selects, counsels, and disciplines staff and maintains appropriate documentation.
Establishes performance expectation, assesses employee performance, gives feedback and conducts yearly performance appraisals with quarterly reviews.
Consistently & fairly interprets and enforces Human Resources' policies & procedures and refers to appropriate person if necessary.
Other duties as assigned.
