St Mary Medical Center, a member of Trinity Health Mid-Atlantic is seeking a Crisis Worker to join our Emergency Department! This is a Full Time position with 4 - 10 hour shifts from 1:
00 PM to 11:
30 PM with a weekend requirement.
In collaboration with Emergency department and care coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient's health care needs in the emergency department.
Through communication and identification of available resources, participates in the promotion of quality outcomes, improving patient experience, reducing readmissions, and cost-effective care delivery.
Provides crisis intervention services by providing discharge planning for medically stabilized patients with behavioral health problems who present extraordinary disposition barriers.
Provide support for warm handoff to care for those in our community with evidenced based treatments for mental health and substance use disorders.
Requirements Minimum of one (1) year experience in a hospital or health care facility Hours are 6:
